A COMPARISON OF OPEN ONLAY MESH VERSUS INLAY MESH REP AIR TECHNIQUE FOR ANTERIOR ABDOMINAL WALL HERNIA 1N FIJI

Background: Two types of anterior abdominal hernia repair is performed in Fiji. On lay and inlay mesh repair using non absorbable synthetic mesh with various outcome and complications.
Aim of this study is to compare clinical outcomes and complications associated with open mesh repair of anterior abdominal wall hernia in Fiji.
Method: A retrospective study utilizing hospital registry to identify patient that had undergone anterior abdominal wall hernia repair between 2002 and 2009 at public hospitals in Fij i. Patient characteristics and clinical information such as type of hernia, technique of repair, surgeon's seniority, length of stay, early and intermediate term complications were analyzed. Follow up was conducted to investigate recurrence and
longevity, and patient satisfaction.
Results: Total of143 patient were grouped as inlay (n-73) and inlay (n-70). Analysis revealed 46.6%9 (n-34) and 16% (n-ll), [CI: 0.7-5.2] recurrence of hernia, respectively, for on lay and inlay group. P-value was 0.1. On lay mesh had higher complication rate 17.8% (n-13), compared to 10% (n-7), [CI2.1-10.3] for inlay mesh repair.
Conclusion: Although in this study revealed no difference in term of durability and recurrence. Inlay mesh placement is better than on lay mesh for abdominal wall hernia repair due to less post operative complications and high patient satisfaction. Extreme care is advised during the peri-operative period to avoid complications like seroma formation and wound infection.
